The office is open to accept applications Monday through Thursday.  If you need an exception to this policy please inquire with the front desk.

HUD 9886 Authorization Privacy Act Notice   (You will need to print one of these forms for each person 18 years or older.)  Each person 18 years  or older signs one of these forms and submits with the application.)

Authorization for Release of Information   (You will need to print one of these forms for each person 18 years or older.)  Each  person 18 years or older signs one of these forms and submits with the application.

HUD 214 Declaration of Citizenship   (You will need to print one of these forms for EVERY person in the household and sign it. )  All these forms need to be submitted with the application.

08 CRIMINAL HISTORY-PH 4-8-14 not signed You will need to print one of these forms for each person 18 years or older. )  Each person 18 years or older fills out this form and submits with the application.
